If it has to be in the user device, then there are chances that user may
get to know about this. User can always monitor the packets flow from
his device.

Its not, unless the message is intercepted before it is encrypted;
implying that someone would have to insert voice traffic monitoring
software upon the targeted user's VOIP access device.
